When you’re having an issue with your ostomy, it can halt your whole life until you get it solved. For many, access to a Nurse Specialized in Wound Ostomy & Continence can be tricky. Or sometimes issues crop up on the Sunday of a long weekend, or in the middle of the night. So having tools to help you determine what is happening with your skin and how to solve it can be a lifesaver.
Peristomal Skin Assessment Guide
The Wound Ostomy Continence Nurses (WOCN) Society, supported by Hollister, developed the Peristomal Skin Assessment Guide, which is available in both web-based and app format.

It asks you to assess your skin and answer questions about your ostomy and pouching to help guide you to a solution.
There is a library of photographs to help you find the skin condition or pouching issue that best matches your experience – the button is in the top right-hand corner, second from the left.
Solutions
Once you have gone through the assessment process, the tool gives you suggestions on how to manage your issue – there are links to videos, and troubleshooting tips to get you through until you can see a nurse.
